Our professional staff will adhere to high ethical standards to ensure all information collected from you is kept confidential and secure at all times. Information gathered will only be used for the purposes for which you give it to us.
Information may be shared with other heath professional as part of your treatment plan (for example we may write to your doctor, send a referral for an x-ray or consultation with another health professional). Should you not wish this to occur, please inform our staff and we will make a note on your file.
Your personal information will not be disclosed to any other third party unless we are required by law to do so.
De-identified information may be used for research or statistical purposes. No personal information will be disclosed should this occur. If you object to your information being used in this manner, please inform our staff so we can note your request.
Information regarding family members of dependants can only be discussed with the individual’s permission. If the individual is not able to give permission due to age or disability, information can be discussed with an immediate family member or legal carer.
You may access any information we hold relevant to yourself. A small administration fee may be payable if you require copies of the file. If you feel any information is incorrect, please inform us so that it can be noted on your file.
We are sensitive to the safe keeping of your personal information at all times. If you feel any of these issues have been or are being breached by any of our staff members, please inform us immediately so we can discuss your concerns.
